Please fill in the delivery notes correctly according to the sample below.
For a smooth passage through the gate, the weighing process and to meet the legal conditions for the transport of wood, it is necessary to fill in the following data on the delivery notes.
If the Delivery Note is not filled in according to the instructions below, the delivery of wood in the MSCP does not have to be taken over!
- The printed Delivery Note must contain in addition to the printed data, also:
- Signature of driver
- Volume of loaded wood in prm / m3
- Measurement dimensions - length, width, height of straightening and number of straightenings
- Delivery list number of State Forests (in case of deliveries from State Forests)
- Signature of the person who issued the Delivery Note
- In case of an incorrect / non-standard GPS record, the driving record must be filled in
- The handwritten Delivery Note must contain, in addition to the printed data, the following duly legible data:
- Number of delivery mail
- Contract
- Date of loading
- Scheduled arrival
- Wood species
- Type of transport
- In the oversized box, indicate whether the delivery is oversized or not
- Name of the transport company
- Driver's name
- Signature of driver
- Car registration plate
- Trailer registration plate (if it has GPS)
- Name of the Place of Delivery
- Consigned assortment and type of wood
- Loader Company name
- Car registration plate (loaded car)
- Marking of the whole certification number, Certification stamp
- Volume of loaded wood in spatial meter / m3
- Measurement dimensions - length, width, height of straightening and number of straightenings
- Delivery note number Sate Forests (in case of deliveries from States Forests)
- Signature of the person who issued the Delivery Note
- In case of an incorrect / non-standard GPS record, the driving record must be filled in
- It is forbidden to cross out and rewrite data in the delivery notes
- We ask the supplier of Sate Forests and the drivers who take deliveries from the Sate Forests to stick the adhesive labels - consignment labels framed in gray in the lower right corner of the Delivery note.
